PERSPIRE: Preventing rehospitalization in lung transplant recipients through mHealth utilizing individualized rehabilitation prescriptions

Brief description of study

Lung transplant recipients have the highest risk of readmission after the index admission among solid organ transplant patients. We hypothesize that targeted, individualized rehabilitation plans will reduce the risk of unplanned readmission in the first year after lung transplantation.To address this hypothesis, we will conduct a prospective cohort study of lung transplant recipients at the University of Pennsylvania and the University of California San Francisco, utilizing the Aidcube application for targeted rehabilitation program development.
